Face lift scars are the signature of the plastic surgeon performing the facelift.  Consequently, it is very important that they be as invisible as possible. Too often, scars are an obvious sign of a previous face lift. It is shocking to see so many celebrities walking around Los Angeles and Beverly Hills with bad scars. Even on the big screen you can see the evidence of a face lift on some actors. Some of that has to do with the old fashioned “horizontal pull” technique that leaves the patient with a “wind tunnel” look, and some of that has to do with poor quality scars.

An unattractive scar these days is completely unjustifiable in most situations.  Some skin types tend to keloid, which is not something the surgeon can control, but a true keloid is rare.

Bad face lift scars are the result of multiple errors in planning and executing the face lift closure.

Planning the incisions is very important. Factors that influencing the plan are the shape of the hairline, how much excess skin is removed, whether the patient is  male or female, and the list goes on. An experienced plastic surgeon can predict the affect these factors will have on the look of the scar and adjust his technique accordingly.

The most important factor of all is the amount tension that is placed on the scar at the time of closure. The tension is directly proportional to well the scar is going to heal.  Too much tension on the closed incision can cause the scar to look wider and more spread out.  Too little tension and there is a good chance that the laxity of the skin will return in as little as a few months.  If you invested time and money in having a face lift, you will not be very pleased to see sagging skin coming back so quickly.

Tension, like volume replacement, is necessary to deliver a long lasting result. That is the reason why I pioneered the VIP closure. Instead of discarding the excess skin at the end of the face lift procedure, I use a portion of it as anchor points in two critical areas of the closure (front and back). By doing this, the tension is shifted from the area along the suture line to the two anchor points creating a tension free closure that will allow the scar to heal well without compromising the longevity of the face lift results.

Another telltale sign of having a face lift is the position of the ear lobes and the cartilage of the tragus.

Often, the tragal cartilage gets blunted during a facelift or the ear lobe is left attached to the skin (Pixie Ear) and pulled down, both leaving an unnatural look.  Placing the axes of the ear lobe in a posterior position of 10 degrees avoids both these situations.

Unfortunately  these errors frequently happen because the plastic surgeon is tired at the end of the procedure and leaves these important tasks to an assistant. I only perform one surgery a day so I can focus 100% of my energy for the duration of the procedure and I never allow an assistant to step in for me.  I do everything from the first incision to the last stitch to make sure everything is done to my standards.

Bad face lift scars are often avoidable with accurate planning and execution.  You don’t have to go around advertising that you’ve had a facelift.

Most of the patients in Beverly Hills and the Los Angeles area considering lip rejuvenation make a common mistake: they focus mostly on the lip itself and its volume. They perceive the lip as too small and they are seeking to make it fuller with all sorts of injectables and fillers. This leads to unnatural results.

What causes a lip to age is complex and many factors are involved. The lips are surrounded by a round muscle called the orbicularis ores and its constant action causes the lip itself to thin out.

Furthermore the lip itself undergoes the same changes that the rest of the face does: smoking and sun damage are contributing factors in the aging process. Also some patients have a tendency to pierce their lips in a certain way causing more wrinkles.

One of the least desirable affects of the aging process on the upper lip is that the distance between the nostrils and the lip elongates, and less of the upper front teeth show. The combination of the lengthening of the upper lip and the deflation of the lip itself creates a very unattractive and older look.

Most of the patients, as I pointed out earlier, focus their attention on the volume, completely missing  the increasing length issue between lip and nostrils. When they make the lips bigger with a fillers, they are still unhappy with the results.  Actually the lip looks worse.

Until the lip distance is shorten the lip will never look good.

The lip lift is the answer to this problem. The lip lift is a rather simple procedure which can be performed under local anesthesia. Trough a small gull shaped incision under the nostrils the excess skin is removed. The orbicularis muscle is then shorten by anchoring it to the periosteal of the base of the nose to assure long lasting results and less scar migration. The incision is then closed with absorbable sutures. If volume on the lip is needed, the lip lift is often combined with fat grafting or fillers like Juvederm or restylane to restore a more youthful appearance.

One thing all males have in common is the desire to look “rugged” with a few wrinkles remaining.  Basically, they want to look healthy and well rested.

Hiding the scars it is also of paramount importance for male patients – and not just in Hollywood.  It’s more of a challenge too, since male patients tend to have much less hair than female patients.  There are loads of examples of bad face lift scars out there, even among the most famous actors. That is the reason I introduced the VIP (tension-free) closure which allows extra support around the incision – dispersing the pull- tension at the anchor points and producing a much more discreet scar.

Avoiding the” done look” is essential and that is the reason the vertical face lift is such a hit among Hollywood celebrities.  The combination of face lift with volume replacement, fat grafting and stem cell enhancement or the cell-enhanced face lift is the most technically advanced face lift available in Hollywood and Beverly Hills.

From a patient’s prospective, it is always difficult to distinguish  one face lift technique from another. Very often the difference is minimal and other factors end up influencing the patient’s choice of surgeon. In fact, most of the time patients have no clue what technique the surgeon uses, and their choice is based mostly on trust, affability, and things like media exposure .

1. Dr. Calabria uses a vertical vector instead of a horizontal one.

This is the most important factor. 99% of all face lift techniques use a horizontal vector.   This is the reason other surgeon’s results are, at best, unnatural. Gravity pushes down tissues in a vertical direction and the tissue needs to be moved back in exactly the opposite direction. This requires that the framework of the face, underneath the skin – specifically the muscles and fat – needs to be repositioned in a vertical direction. Why doesn’t every surgeon do it that way??  It requires knowledge and experience, it is time consuming and technically challenging.  However, it makes an enormous difference!

2. Dr. Calabria uses tumescent anesthesia

This is also very unique.  Filling up the space underneath the skin with diluted local anesthetic solution allows Dr. Calabria to do even the most complicated procedure like a face lift under local anesthesia. It also minimizes the post operative bruising and swelling as well as decreasing the pain after surgery. Most plastic surgeons use a much smaller amount of anesthetic and they do the face lift under general anesthesia.
