Breast Augmentation: Incision & Implant Placement Options

If you have decided to undergo breast augmentation surgery, it is important to learn about all the options available for your procedure. Along with choosing your implant size and type, you will also want to think about the incision location and implant placement options. Based on the implant type chosen (saline or silicone gel), incisions can be placed along the breast crease, around the nipples, in the underarm, or sometimes in the navel.

Since silicone gel implants come pre-filled, they generally require a longer incision than saline implants. The most common incision type for silicone implants is along the breast crease. This allows for a slightly larger incision to be made (scarring is minimal) for the implant to be inserted with greater ease. Depending on the degree of enlargement desired and the patient’s particular anatomy, the incision for silicone implants can sometimes be made around the areola.

Saline implants are filled after they are inserted into the breast, which allows for a greater variety of options for the location of the incision. These include around the areola, under the breast in the crease, through the navel, and under the arm. Regardless of incision location, scarring is minimal and easily hidden by clothing and bathing suits.

Once you have chosen the best incision location to be used for your breast enhancement, you will need to decide where in the breasts to place the implants. Implant placement options include under the breast tissue but above the muscle (subglandular) or beneath both the breast tissue and the pectoral muscle (submuscular). Depending on the amount of breast tissue present, the implant may be best placed under the muscles to reduce the appearance of rippling. In addition, Dr. Calabria also offers the “dual plane” technique in which the implant is partially covered by the muscle. This option reduces risk of capsular contracture and rippling.
