Traditional Facelift Problems
All these problems need to be addressed and corrected in achieving a satisfying result.
Wind Swept Look
The direction of the pull in traditional face lift is horizontal, when gravity acts more in a vertical direction. With aging this" lateral sweep" gets worse because the deep structures of the cheek which were not corrected with the conventional facelift, keep on falling down creating disharmony with the "lifted" skin, making more evident the lateral pull toward the ear, specially in photo damaged skin where the natural lines of the face are redirected into an unnatural pattern.
Hollow & Rounder Eyes
Most plastic surgeons perform a blepharoplasty, both upper and lower to improve the periorbital area. The upper blepharoplasty invariably leaves the patient with a rounder, unnatural eye shape. The lower blepharoplasty, by removing the so called "bags" fat, also causes hollowing and contour defect of the lower periorbital areas.
Low Eyebrow & Aging Foreheads
The forehead rejuvenation is overlooked and often left undone and does not match with the more youthful face creating a disharmony in the face.
Hyperthrophic Scarring & Pixie Ears
The earlobes are pulled down too far or too far forward: first give away sign then surgery was done.
Traditional facelift causes a flattening effect on the cheek by pulling tight the skin "only", in the wrong direction.
With traditional face lift surgery (“rhytidectomy”), facial fat is removed and excess skin is pulled in a lateral direction. The unnatural, “wind swept” appearance of many face lift patients is caused by this technique.
A newer, less invasive method of facial rejuvenation, Vertical Facelift surgery is performed through two discreet incisions. With the careful placement of sutures in a strategized position, the face can heal more rapidly and appear more youthful than with a traditional face lift.
Wind Swept Look
The most difficult problem to solve. The deep tissues need to be repositioned in a more vertical vector; this is accomplished with Dr. Calabria's Vertical Face Lift. Dr. Calabria studies old images of the patient in their twenties and recreates a more youthful look by reversing the anatomy to the original shape. The cheek, fat and muscles are repositioned vertically. In addition, micro fat grafting is injected in the cheek area to "volumize" the face.
The lower eyelid hollow needs to be corrected by repositioning the orbicularis muscle and by filling the space with micro fat grafting to recreate a fuller lower eye lid. Routinely a cantopexy is performed to reposition the lower lid which is usually pulled down causing the so called "scleral show".
The forehead aging is correct with an endoscopic brow lift: this procedure will not only reposition the eyebrow but it will also improve the rounder shape of the eyes, making them more open and more almond shape.
The suspenders face lift will eliminate the tension at closure lines and it will minimize previous scarring.
Flattening Of The Cheeks
Redirecting the vector of pull to vertical and "volumizing the face" will be crucial to correct this problem.
Achieving balance, harmony and a natural look by addressing all the problems caused by traditional facelift surgery and correcting them is essential: the vertical face lift along with the breakthrough procedures we have described are the solutions.
Sometimes this occur after months from the procedure, more often after a few years, depending of the face lift technique and the type of skin of the patient, her/his racial and genetic back ground , diet, smoking habits and overall state of health.
At this time the only solution available to correct the recurrence is to perform another face lift with the same risks, chances of complications and down time of the first operation.
A new and revolutionary concept and technique has been developed by which not only the duration of the original face lift is longer but and fore most the need of a second face lift operation maybe obsolete: this is called the Adjustable face lift.
Traditionally, when a face lift is completed, the skin is pulled tight, the excess skin excised and the edges are sewn up in front and behind the ears. These may result of unsightly and hypertrophic scarring from the tension applied in the closure in the effort to create a tighter look. Later on, after the swelling has resolved, there may be a recurrence of the skin laxity which, unfortunately, may occur sooner then later.
The Adjustable Face lift is developed in a way by which at a later day when the skin laxity reoccurs, trough a small adjustment done under local anesthesia, the skin could be repositioned in a more youth full fashion, avoiding, therefore, the morbidity of a longer operation.
How Its Performed
At the point of redraping the skin, at the completion of the face lift, the excess skin is marked in front and back of the ears. Traditionally this excess skin is simply excised and then the edges brought together with sutures. In this procedure, instead of the full skin, only the epidermis or top layer of the skin is excised, leaving the dermis and subcutaneous tissue fully intact, both in front and behind the ears. Two suspenders are, in fact, created, made not of suture material but of your own natural tissues. Then a few centimeters apart, in the scalp, two pockets are created trough two small incisions, small tunnels are developed, and the suspenders (or flaps) are inserted in to the tunnels and brought up at the level of the new pockets previously created. Then two important steps follow: one non absorbable suture is placed trough the suspenders tissues and left in the pocket untied, another suture, this one absorbable, is used to anchor the suspenders in place to the fascia. The second suture along with the suspender carries all the tension of the procedure which is usually carried by the skin edges creating bad scarring, allowing the incision to heal with minimal scarring. Months or years from now the aging process and gravity is going to cause a recurrence of skin laxity and jowls (with this technique the recurrence is going to happened later then a traditional face lift), but nevertheless when it happens, all is need it to correct it is to open the scalp incision under local anesthesia and tie up the suture connected with the suspenders: this will correct the residual laxity in the face and neck. The down time is minimal and the need of a second face lift is avoided
What is this procedure accomplish is , basically, put suspenders on a face lift: at a later date, when need it, these can be simply tight up therefore reversing the signs of aging by a quick, easy, no down time , local procedure.
For decades plastic surgeons have been looking to restore a more youthful look on the face. Surgical techniques have been improving, newer lasers have been used in order to achieve a goal that seemed unreachable.
Comparing the post-operative results with the pictures of the patients when they were younger, it appeared that there was something missing: a younger face had more volume and the skin just looked fresher.
Dr. Calabria always thought that with the advent of stem cell research, plastic surgery could beneficiate from it. Using stem cell as a mean to jump start the regenerative process in our own body was definitely a very attractive proposition.
We knew that the fat in our body contained adult stem cell along with other regenerative cells. Dr. Calabria has been using fat grafting ( therefore containing adult stem cells) on the face with the distinct purpose to rejuvenate the skin and adding volume.
But was not till the develop of a new device, the Celution Device, that facial rejuvenation could taken to the next level and finally create a really amazing result.
Dr.Calabria has called this new way of the future the "cell-enhanced face lift". The (stem) cell-enhanced face lift is a new procedure used to volumizing the face and create a more youthful look.

Adding volume to the face has always been Dr. Calabria's goal. First with the Vertical Face lift, by moving the patient's own tissues in a vertical direction and therefore repositioning it to the original and more natural position, Dr Calabria has been creating a more youthful look. And now with the (stem) cell-enhanced face lift, Dr. Calabria think he has definitely found the answer.
The (stem) Cell-Enhanced face lift not only restores the volume in the face but also adds regenerative cells which are believed to be beneficial to the rejuvenation process.
Dr. Calabria has developed a technique by which the cells are injected into the SMAS and underneath the facial musculature to create a more youthful volume adding result.
The (stem) cell-enhanced face lift combines soft tissue that is transferred from one part of the body to the face with the patient's own adipose tissue-derived stem cells and regenerative cells.
The technique first consists of harvesting the cells ( Usually from the lower abdomen). Liposuctioned fat is rich with regenerative cells which include adult stem cells, blood vessel producing cells, growth factor secreting cells. Once the fat is harvested, it is treated through a device to isolate the cells.
First, adipose tissue is taken from the patient by a low volume tissue collection procedure. Next, the collected tissue is placed in the Celution® Device, which processes each patient's tissue with a single-use, application-specific consumable set that easily attaches to the Celution® Device.
The Celution® Device liberates the stem and regenerative cells from the adipose matrix; the cells are then separated, washed, and concentrated in the collection container. This real-time processing takes place in a closed environment to minimize the risk of exposure to contaminants and can be completed within the timeframe of a single surgical procedure.
Next, a syringe is used to collect the cells that are to be redelivered to the same patient. The cells may be injected directly into a site or implanted with a delivery matrix or scaffold to improve performance, such as combining the cells with an adipose tissue graft in reconstructive surgery procedures.
Following re-infusion into the body, it is thought that environmental cues from the damaged and surrounding tissue guide the stem and regenerative cells to the area of damage and help facilitate a natural healing response. The cells may respond by a variety of mechanisms, which promote tissue survival, graft retention, and/or differentiation of progenitor cells into the appropriate cell type.
Then, during the face lift, the regenerative cells tissue embedded in fat is then gently placed with a blunt cannula in layers of facial tissues like the SMAS, under the muscles, etc.... Once the fat is placed, symmetry is checked and then the skin is redraped and approximated, the excess skin excised and then anchored in a vertical direction and closed in layers. The key is to place this regenerative tissue in a precise location in order to enhance the volume of the face in areas previously volume deprived.
After the cell-enhanced face lift, the patient exhibits a dramatic improvement not only in the underlying soft tissue contouring of the face but the skin itself. The growth factors contained in these cells induce the skin and the other tissues to produce more cells of their own by basically initiating a signal to the local stem cells as well as the transplanted adult stem cells to restore them self and multiply.
Others techniques of stem cell face lift inject the cells directly in the face,( without surgery) but in Dr. Calabria's opinion is not as effective in voluminizing the face that the cell-enhanced ( stem cell) face lift and it does not address the laxity of the skin.
Also the cells have not been isolated with the celution device which Dr. Calabria thinks is essential to the final result.
The regenerative cells, by secreting growth hormone and other factors, are also beneficial to create a more youthful look, diminishing the aging process. The procedure restores the youthful contour and shape of the face as well as skin tightness and evens out color irregularities caused by aging and sun damage.
The cell-enhanced (stem cell) face lift is the new frontier of face lifting and Dr. Calabria is definitely writing a new chapter in facial rejuvenation.
The Celution device is not available in the U.S. yet but has been approved for clinical use in Europe and it is available in Dr. Calabria's Milan and Rome locations and he will bring the technology to the U.S. as soon as next year.
