Dr. Calabria's philosophy about breast augmentation is based on achieving natural result by using state of the art techniques and tailored made implants specifically designed to reach that goal.

Dr Calabria is well known to create a very natural breast for an active patient: his sophisticated clientele wants to go unnoticed, no matter what area of the body or face they choose toimprove.

More natural breasts tend to cause much less problems in the long run then very large implants. Larger implants, even if at the beginning may look attractive, usually with time tend to stretch the skin envelope and cause the breast to drop. Large implants may also cause rippling which is a very noticeable problem. Once this happens it is very difficult to fix the problem and the patient may end up needing a lift with the obvious, visible scars. Large breast may also cause discomfort due to the weight.

Dr. Calabria uses the latest technique to insert the implants called "dual plane" where the muscle in part covers the implant there fore there is less incidence of capsular contracture and rippling.

A variety of shapes are available to the patient including round, tear drop, low, moderate and high profile depending on the anatomical framework of the patient.

Dr. Calabria breast augmentation technique is based on meticulous dissection, minimal blood loss, minimal trauma to the surrounding tissues which in turn equates to less pain after surgery, a more rapid recovery and less long term complications such capsular contracture.

Breast Revision

Dr. Calabria feels that breast augmentation is a procedure with a high degree of satisfaction. In fact, from a recent survey, it is shown that despite some of the complications due to the surgery and the implants, more then 90% of women are very happy with their decision and would recommend the surgery to a friend.

Undoubtedly, there are some problems that can lead to the need of a breast revision.

Capsular contracture

Capsular contracture is the formation of scar tissue around the implant severe enough to cause hardness of the breasts and pain. Capsular contracture can also cause some deformity of the breasts and change in their shape. If it happens, it could require a second operation to remove the scar tissue.

Rippling

At times, especially if there is not much breast tissue to cover the implant, the implant can be palpable or worse the edges could be visible creating a rippling effect. The solution is to replace the implant under the muscle.

Deflation or rupture

The implant can leak (both silicone and saline). Obviously if it happens, it should be replaced.

Recurrent ptosis, or sagging of the implants

Often, especially if the implants are too large, the weight of the implants will stretch the skin and cause ptosis of the breasts.

This will require a second operation or mastopexy (lift). If the ptosis is severe, extensive incisions may be required on the breast to correct the problem.

Implants malposition

It is a common cause of revisional surgery. The implants are asymmetric; they are placed too lateral or too medial.

Breast revision could be a challenging operation depending on the cause triggering the problem. Dr. Calabria feels that a lot of these problems can be avoided by planning the first operation very carefully. The size of the implants should be in proportion to the frame of the patient; during surgery is important to use a meticulous dissection technique to avoid any excessive trauma to the tissues and bleeding that could trigger the formation of scar tissue later. Planning the revisional surgery is also very important because the implementation of an extensive lifting procedure in the presence of a decrease vascularity of the breast tissues due to the presence of scar tissue can lead to further complications.
